Please print and fill out the medical history form before arriving for your visit. Make sure you sign the bottom of the form.
It is recommended that ALL patients have either their eyes dilated or the OPTOMAP digital retinal scanner performed at each comprehensive eye examination. The OPTOMAP is painless, quick, and does not require drops and cause blurred vision. (Please note that some patients may need to have their eyes dilated in addition to the OPTOMAP if required by their doctor).
